Basically "shit happens" - it could have just been that the worm motor was jammed or something. All LiteOn drives (at least on the old style 360s) seem to suffer from the getting-stuck-at-the-top issue, btw. Take a 1mm washer and place it at the far end of the laser's rail, then glue it in place (glue it to the bracket where the rail is held in place). The laser won't ever get past the "point of no return" if you do that, and it shouldn't have any issues playing discs either (until the laser or motor dies).
