Unplug all your controllers to confirm they're not at fault.

If not, does this fit the bill?

Also, my whole controller goes dead.  I can't "undo it by playing a music file", as I have no more control.

When the ticket's sorted, it'll have a tag stating it's closed. Though whether it applies to you depends on how similar your symptoms are. In my case, I know how to specifically trigger or avoid it; the question is if my methods apply to you.

You might like to try posting up a debug log on PasteBin and linking to it here. To do this, hold X+Y as XBMC starts, trigger the error, then grab the log file (stored in your XBMC directory) via FTP.
